Dear Hall of Fame Inductee,
On behalf of the United Fellowship of Martial Artists, we would like to congratulate your nomination into the Hall of Fame. Your name has been submitted to us and approved by the Hall of Fame Directors.
The Hall of Fame Banquet is a (semi formal affair) being held on Saturday, Oct. 22 at 6pm at The Joy Tsin Lau Restaurant 1026-28 Race Street (China Town) Philadelphia, PA 19107 USA Tel. (215) 592-7227. We also offer a Junior Hall of Fame Award for children ages 17 and under. The inductee package includes a Hall of Fame Certificate or Trophy, a United Fellowship of Martial Artists (UFOMA) Patch, and a lifetime membership Certificate.
All guest banquet tickets are $65. All fees are due by Oct. 10, 2011. If you are not able to attend this year, see you at next Banquet.
Please complete and submit the Hall of Fame Acceptance Form provided in page #2 of this letter along with 2 passport photos and a short martial arts biography/resume to us by Oct. 10, 2011.
If you would like to nominate someone whom you feel is worthy of induction to the Hall of Fame, please fill out the nomination sheet provided in page 4 of the attached letter and submit it to us for consideration by Sept. 30, 2011. The Hall of Fame Award Categories are listed in page 5 of the attached letter.
I am truly honored to have had the opportunity to notify you of your induction. This is one of the highest honors bestowed to a martial artist. We look forward to seeing you at the Hall of Fame Banquet on Oct. 22, 2011.
